Reducing Readmissions for CHF & COPD

The Oregon Health Care Quality Corporation is one of 16 communities selected to continue participation in the Robert Wood Johnson Foundation’s Aligning Forces for Quality initiative in 2011. As part of this next phase of work, Quality Corp will coordinate community activities focused on improving care for Oregonians with low back pain and reducing hospital readmissions for patients with congestive heart failure (CHF) and chronic obstructive pulmonary disease (COPD).

Quality Corp is collaborating with a wide variety of Oregon health care organizations on efforts to reduce hospital readmissions for CHFand COPD. Although there are many innovative efforts in Oregon to improve this area of care, Quality Corp sees an opportunity to bring stakeholders together to share best practices and align efforts.
