This is a checklist of roles that a Patient and Family Advisor had the option of taking on and indicated which ones were of interest or not on the form.

In June 2010, Oregon Health Care Quality Corporation brought together health care professionals, community leaders, patients/families, community foundations and administrators for a workshop to discuss and learn about organizations and individuals that were transforming health care through patient involvement and decision-making in health care leadership. It was a great opportunity for attendees to share information and explore common issues/strategies used to improve patient/family involvement.
