Oregon has been selected as one of fourteen regions to take part in the CPC+ initiative that will start on January 1, 2017. Practice applications for CPC+ are now open on the online portal through September 15, 2016. The CMS website has practice application questions, along with additional important information for practices and vendors regarding Health IT requirements (Appendix B and C from the original RFA).
This webinar is open to all interested practices in Oregon, including those not currently participating in CPC.

David earned his BA in Economics and his MD from Washington University in St. Louis. He then completed Internal Medicine residency at Oregon Health & Science University, and earned a Master's in Medical Informatics and Health Services Administration from the University of Utah.
Broadly, David’s interests lie in care management, coordination of care, collaborative care, chronic disease management, quality, and the requirements of clinical information systems to improve and support these areas. His current primary concentrations are Transforming Outcomes for Patients through Medical home Evaluation & re-Design, TOPMED (funded by The Gordon & Betty Moore Foundation), Risk Stratification in Primary Care (funded by the Commonwealth Fund and AHRQ) and further dissemination and evaluation of the Care Management Plus project (initially funded by The John A. Hartford Foundation). David is interested in policy and payment reforms to help provide better coordinated, patient-centered care and support efficiency in the health care system. He was chosen as the New Investigator of the Year by the American Medical Informatics Association in 2007.
Ron Stock MD, MA
Clinical Innovation Advisor,
Oregon Rural Practice-Based Research Network (ORPRN)
Dr. Stock is a geriatrician, family physician, clinical health services researcher, and currently a Clinical Innovation Advisor to the Oregon Rural Practice-based Research Network (ORPRN) and the Oregon Health Authority Transformation Center. A graduate of the University of Nebraska College Of Medicine, Dr. Stock completed his residency and faculty development fellowship in Family Medicine at the Medical University of South Carolina and University of North Carolina-Chapel Hill and has a Certificate of Added Qualifications in Geriatric Medicine. He is currently an Adjunct Associate Professor of Family Medicine at Oregon Health & Sciences University. Before joining OHSU in 2012, he served as Executive Medical Director of Geriatrics and Care Coordination services, and Medical Director of Education & Research at PeaceHealth Oregon Region. Over the past 2 decades with support from public and private non-profit grants, he has dedicated his career to improving the quality of healthcare for vulnerable populations with complex care needs, focusing on redesigning the primary care delivery system for vulnerable elders using an interdisciplinary team model. He has been past faculty to the Oregon Comprehensive Primary Care initiative and has participated as a member of the Institute of Medicine Best Practices Innovation Collaborative on Team-Based Care, most recently as a member of an IOM Task Force to explore the role of “patients on teams”.
