Patient- and family-centeredness cannot be attained nor sustained solely through individual interactions that occur in single health care encounters. To impact real change, health care organizations must build internal infrastructures that elicit honest interactions with patients and families in decision-making settings. Health care organizations will need training, education, and executive will. Partner for Quality Care seeks to facilitate this revolution by raising awareness, piloting intensive technical assistance to embed patients and families within organizational infrastructures, and sharing our lessons both locally and nationally.

Patient- and family-centered care is an innovative approach to the planning, delivery, and evaluation of health care that is grounded in mutually beneficial partnerships among health care providers, patients, and families. Patient- and family-centered care applies to patients of all ages, and it may be practiced in any health care setting.

Organizations selected for technical assistance will be announced the week of August 4, 2010.
