Oregon Transitional Care Collaborative

The Oregon Transitional Care Collaborative is a statewide partnership of health care and community service organizations working to improve patient transitions between care settings. The goals of the Collaborative include:

• Promote local partnerships among health and community services providers to ensure quality transitions between providers and enable patients and their caregivers to participate more fully in their health care.

• Encourage the use of quality improvement concepts and tools to change systems within facilities and across the local care continuum.

Community teams, composed of representatives from hospitals, skilled nursing facilities, home health agencies, physician practices, and Area Agencies on Aging, are working together to forge local solutions to reduce the unnecessary rehospitalizations or ED visits that result from poor transitions. Counties participating in the collaborative include:

• Lane County
• Clatsop County
• Yamhill County
• Marion/Polk Counties
• Linn/Benton Counties
• Jackson County
