Hospital Collaboratives
To help improve the quality of health care, the Oregon Health Care Quality Corp coordinates and convenes collaboratives that bring together hospitals across the state and across the nation to develop and share quality improvement tools, strategies and lessons learned to improve care delivery.
Oregon Transitional Care Collaborative
The Oregon Transitional Care Collaborative is a statewide partnership of health care and community service organizations working to improve patient transitions between care settings. The goals of the Collaborative include:
Promote local partnerships among health and community services providers to ensure quality transitions between providers and enable patients and their caregivers to participate more fully in their health care.
Encourage the use of quality improvement concepts and tools to change systems within facilities and across the local care continuum.
The Hospital Quality Network is a nationwide learning network of over 130 hospitals working together to improve the quality and safety of patient care in 17 communities. Over a quarter of Oregon hospital (15) are participating in the program – an incredible turnout that demonstrates the commitment of Oregon hospitals to providing high quality care. Participating hospitals will develop and share quality improvement tools, strategies and lessons learned to improve care delivery in one or more of three areas: reducing readmissions, increasing throughput, and/or improving language services.
The program is funded and led by the Robert Wood Johnson Foundation as part of their Aligning Forces for Quality Care initiative; Oregon Health Care Quality Corp and Oregon Association of Hospitals and Health Systems will coordinate the program in Oregon.
