Health Literacy Universal Precautions Toolkit: Sponsored by the Agency for Healthcare Research and Quality, this extensive toolkit provides step-by-step guidance for assessing your practice and making changes so that you connect with patients and caregivers at all literacy levels. It’s designed to be used by all levels of staff.
Advancing the Practice of Patient and Family Centered Care: Developed by the Institute for Patient and Family Centered Care, this resource includes a section called “Where Do We Stand?” that you can use to assess how well patient and family-centered approaches are embedded in your organization’s culture.
Personal Health Record: The Lane County Transitional Care Collaborative has developed a Personal Health Record based on a component of the Care Transitions Intervention developed by the University of Colorado at Denver.
